Overview:

Additional Patient Info Form allows labs to capture comprehensive patient information at the time of registration, ensuring accurate and relevant data collection. Customisable forms make it easy to capture and manage additional details for each patient, enhancing clinical workflows.


How to configure/set-up these forms?

Step 1: Go to Admin -> Lab Forms Management -> Additional Patient Information

Here is where you will see all the configured forms (if any). To configure a new form, you will go to "Add New Configuration". and create a new form of your choice.

- You can also choose to copy (duplicate) an entire existing form, and create a new form with whatever required changes are there to do, this ways, you can have minimal time to set-up a new form if there are same/similar configurations you need on the new one.

- If you wish to discontinue any form, you can simply disable it, and enable again if it is required again.

Step 2: Add a New Configuration

- At first in basic information, you will give the form a title, and add a description (if required), and a form code (if required)

- Post which you will go to Form Components, this is where all the questions of the form will be configured. You can choose amongst a variety of question types- text, date, dropdown, radio-buttons, etc.

- Once you are done with all the configurations of required questions, you will then save the form.

Step 3: Map the configured forms in Registration Settings -> Additional Patient Info

- Choose which forms to display for different types of registrations (Appointments, Home Collections, Walk-Ins, B2B Orders) and select a default form to be displayed among all mapped forms.

- You can also choose to disable the form from here temporarily from here if it is not required.


Please note: Lab Forms Management as a feature has to be enabled for the lab, to be able to use this capability. Please consult sales team before adding it to any lab.
