What is Additional Patient Information?
Additional Patient Info Form allows labs to capture comprehensive patient information at the time of registration, ensuring accurate and relevant data collection. Customisable forms make it easy to capture and manage additional details for each patient, enhancing clinical workflows.

How to capture responses for Additional Patient Information?

1. Additional Patient Information can be captured via various registration pages:
- Default Registration Page
- Custom Registration Pages
- Appointments Registration Page
- Home Collection Registration Page
- CC Login Registration Page
2. The default form selected in registration settings will be the one that will be displayed by default on the respective registration pages. The other forms added, can be navigated by clicking on the dropdown.
3. The responses will be saved by clicking on:
- For new patients - "register" or "register & bill"
- For recurring patients - "update" or "update & bill"
How to update responses after they have been captured?
Additional Patient Information values can be updated via:
- Registration Page: The values can be updated by searching a patient, and updating, the same way we do for other patient information such as date of birth, age, contact number, etc.
- Patient Update modal: This is the modal we see in operations, appointments, home collections, and this is where we display the values of additional patient info, and can be updated.
Please note: To be able to update values of additional patient info, the user needs to have access of "Update Patient Info".
Where can these responses be printed?
These values can be printed on - TRF, Bill, Reports PDFs, although these are not printed by default, but can be added on request by the customers. To get these values on PDFs, please get in touch with the PDF team.
